I think I may have figured this out. Malwarebytes is blocking the connection to ctmservices.gotdns.ch. It is classifying that as a phishing site. That is breaking the connection when I submit the form. It is probably nothing wrong with your server. Maybe you should check why that server is categorized as a phishing site.
